Instructions: 

  • Either order form will open in a new window. Please allow a few moments for it to load, especially the fillable version, which requires the free Adobe Acrobat Reader.
  • The blank form can be printed and completed by hand. Enter payment details (if using a credit card, provide all required information). You may either print and fax the form or save the PDF and e-mail it to prolix@prolixlubricant.com.

 

Shipping information:

  • All containers are “special” poly-plastic with pop-up (PC) applicators, except in bulk 16 oz. and larger.
  • Bulk containers over 5 gallons are available by special order.
  • Orders are shipped via UPS or other local carriers unless otherwise specified.
  • Utah residents must add Utah sales tax.
  • Contact us for shipping quotes on 4- or 5-gallon bulk orders or shipments outside the continental U.S.

 

Shipping cost:

  • We charge a reasonable and fair shipping cost as close to our actual cost as possible.
  • For continental U.S. orders, the website shipping cost represents our best estimate.
  • If our actual cost is within $2.00 (up or down), we process the order using the quoted amount.
  • If the cost exceeds the quoted amount by more than $2.00, including orders outside the continental U.S., we will e-mail you a revised cost and request authorization before shipping or processing any credit card.
  • A 10% fee (not to exceed $20.00) applies for any required custom forms for international shipments.

 

Free/discounted shipping:

  • Orders over $200 receive free shipping.
  • Free or discounted shipping may not appear on the online form, but you will not be charged at checkout or, if discounted, will receive a credit against shipping costs.

 

Returns policy:

  • If our product does not deliver the results described in our materials, call or e-mail us. Once we confirm the product was used correctly, you may request an RA number and return your purchase—even empty bottles—for a full refund.
  • To date, we’ve had only two such requests, and not one bottle has ever been returned! That’s a track record we’re proud to share. At PrOlix®, despite all the testimonials, the test we value most is yours.
